No, I think you're fine with just replacing the top layer. 6 inches or so sounds fine. The reason why you want to replace soil is that the nutrients and organic matter leach out of the pots over time, plus the old plants would have used up some of the nutrients. So basically I just try to give the area the plants are going "live" in the new soil. Now if you are going to go from growing flowers to growing veggies or visa versa, then you would want to give them totally fresh soil suited for that type of crop.

One thing that I found useful is when you put the filler in the bottom of the container (like styrofoam, water bottles etc...) to put a piece of landscape cloth over the top and then the dirt. That stops the dirt from mixing with the filler and I find that when I water, it's just water that comes out, and it's not muddy.
